PURPOSE
OF THIS NOTICE
Allsurance
Agency, Inc. has established policies and practices that respect the
privacy of our customers and consumers of insurance. Our current
privacy practices are summarized below. We reserve the right to
revise our privacy policy at any time, and we will provide you with a
revised notice if changes are made that will result in disclosure of your
nonpublic personal information beyond those outlined in this notice.
THE
TYPES OF INFORMATION ABOUT YOU THAT WE MAY COLLECT
We
may collect nonpublic personal information about you from the following
sources:

we receive from you on applications and other forms, such as name,
address, phone number, social security number, driver license number,
and date of birth;

others such as information about your insurance policy's account
balance, your premium payment history, and your bank account number;

we receive from outside sources such as consumer reporting agencies,
including motor vehicle records, credit reports and claim history
reports.

TYPES OF INFORMATION ABOUT YOU THAT WE MAY DISCLOSE
We
do not otherwise disclose nonpublic personal information about our
customers or former customers to anyone, except for routine procession of
transaction at their request and as otherwise permitted or required by
law.
TO
WHOM WE MAY DISCLOSE THIS INFORMATION
We
may disclose your nonpublic personal information, without your
authorization, include:

or organizations that perform a business function for us,

insurance company or broker

regulatory
and law enforcement authorities, such as government offices or courts
which subpoena records,

claims adjusters who are handling a claim involving you or your
insurance policy,

support organizations which gather data to help deter or prevent
insurance crimes,

insurance companies or insurance support organizations in connection
with an insurance transaction involving you, such as the purchase of
reinsurance,

which conduct actuarial or research studies.
OUR
PRACTICES REGARDING INFORMATION CONFIDENTIALITY AND SECURITY
We
restrict access to nonpublic personal information about you to those
employees who need to know that information in order to provide products
or services to you. We maintain physical, electronic, and procedural
safeguards that comply with federal regulations to guard your nonpublic
personal information.
ADDITIONAL
PROVISIONS
You
may see and copy in person, or, ir you prefer, we will copy and send to
you, the personal information about you in our files.
If
you advise us that any of the personal information we have about you in
our records is incorrect, we will reinvestigate and if we agree there was
an error, we will correct it. If we do not agree, you are still
entitled to file a statement with us, disputing the information. we
will send the correction or statement to anyone who received or will
receive the original information.
In
order for us to safeguard your information, your request to see and copy
such information must be in writing and include your name, address, policy
number, daytime phone number, the best time of day for us to call, and a
copy of personal identifications such as your driver's license.
If
you have a policy with us, you may send your request to us by addressing
it as follows: "Attention: Privacy Coordinator" and then mail it
to: Allsurance Agency, Inc., PO Box 10017, Liberty, TX 77575.
Limitations
on Information Disclosed for Marketing Purposes:
We
will not share the following information with others whose only use of the
information will be in marketing a product of service: your medical record
information, your personal information relating to your character,
personal habits, mode of living, or general reputation, or any
classification derived from such information that relates to a claim by
you for insurance benefits, or a civil or criminal proceeding involving
you, and that is collected in connection with or in reasonable
anticipation thereof.
